Hawaii names standing orders as the first of two lawful modes of supervising certified EMT-Paramedics, approved by the Department of Health and by the emergency department chief of a designated MEDICOM hospital.

Hawaii is unusual in that the standing orders are a single statewide document the Department of Health writes and publishes itself (the Hawaii Adult & Pediatric Standing Orders), rather than a per-agency protocol set. The rule authorises standing orders as a supervision mode but does not name controlled substances, which is why the quote is partial; the rule also limits the standing-order mode to life-threatening situations, with the alternative being on-line direction from an emergency physician at a State-designated MEDICOM base station hospital under § 16-85-60(a)(2).
