Training in Illinois disqualifies you from the reciprocity route

Illinois states that reciprocity is only for people who did not train in Illinois. Someone trained by an Illinois EMS system who has passed the National Registry exam is handled by their system coordinator submitting documentation directly, not by applying for reciprocity.

Same shape as the North Carolina bar, but benign rather than obstructive: Illinois routes you to an easier path instead of closing one. Recorded because someone who trained in Illinois years ago and is now licensed elsewhere will otherwise start filling in the wrong form.

A medical director has to vouch for you, or you submit a CE audit instead

The application requires a signed, dated letter from the EMS medical director you last worked under attesting that you are in good standing and current on continuing education. If you cannot get one, you must explain why in writing and, if you have held the licence more than six months, attach copies of all continuing education for the current period with dates, hours and instructor signatures.

Both branches are work. The letter depends on a physician at a job you have left; the waiver depends on having kept every CE certificate. Worth requesting the letter before you leave a post you might later need it from.

Illinois advanced life support care — which the statute defines to include administration of medications, drugs and solutions — is initiated as authorized by the EMS Medical Director under the written or verbal direction of a physician, with written direction being the standing-order form.

Partly supported

The quote carries the 'written direction' half of the value; the drugs half comes from the same subsection's definition of ALS services, which lists 'administration of medications, drugs and solutions' among the care so authorized. Neither names controlled substances, hence partial. Illinois is structurally unlike most states here: authority attaches to the Department-approved EMS System and its EMS Medical Director rather than to the ambulance service, and under 210 ILCS 50/3.55(b) a licensee may practise 'under the written or verbal direction of the EMS Medical Director' with equipment and drugs that 'shall operate only with the approval and under the direction of the EMS Medical Director'. Illinois also uniquely allows the verbal half of that direction to come from an Emergency Communications Registered Nurse rather than a physician.

Does Illinois require its own controlled-substance registration for EMS agencies?

Illinois requires an IDFPR controlled-substance registration for manufacturing, distributing, dispensing, research, euthanasia drugs, and canine odor detection — administering under an EMS System's medical direction is not among the triggering activities, and no EMS or ambulance registration class exists.

Partly supported

The value rests on what the trigger list omits. Section 302(a) is written as an enumeration and the legislature reached specifically for 'purchases, stores, or administers' when it meant euthanasia drugs — so the absence of purchase, storage, or administration from the general clause reads as deliberate rather than accidental. Two supporting features: § 302(c)(4) excuses 'officers and employees of this State or of the United States while acting in the lawful course of their official duties which requires possession of controlled substances', and Illinois routes EMS drug supply through the EMS System's Resource Hospital, which 210 ILCS 50/3.20(b) makes responsible for 'the replacement of drugs and equipment used by providers who have delivered patients to their emergency departments' — the hospital pharmacy is the registrant in that chain, not the ambulance service. Confidence is medium because this is a negative inferred from an enumeration; no Illinois source was found that addresses EMS agencies and controlled-substance registration in terms. Note also that § 302(c)(4) names the State and the United States but not municipalities or fire protection districts, so a municipal EMS agency does not fall inside that particular exemption.
