Every ambulance service that carries controlled substances must hold its own MCSR from the DPH Commissioner under 105 CMR 700.000, on top of DEA registration, and a complete MCSR application is a precondition of EMS service licensure.

Massachusetts is one of the states where the answer is unambiguously yes AT THE AGENCY LEVEL: 105 CMR 170.303 is an EMS regulation addressed to services, not a generic practitioner rule read across. DPH's MCSR facility pages list ambulance services among the facility types that may register and ask them for a drug security policy, a current hospital affiliation agreement and a current OEMS licence. 105 CMR 170.303 also lets a service apply for licensure on proof that a complete MCSR application was submitted, so the two filings run in parallel rather than in sequence. This is an agency obligation; nothing here requires an individual medic to hold a personal MCSR.

Each service that possesses controlled substances and instruments for administration of controlled substances, in accordance with its level of service and the Statewide Treatment Protocols, shall be registered with the Commissioner, in accordance with 105 CMR 700.000: Implementation of M.G.L. c. 94C.
